This Agreement is entered into by and between Alex Sawyer Productions LLC (Coach) and Client whereby Coach agrees to provide Production Coaching Services for Client.
Services & Fee Structure
Services
Coaching is a partnership (defined as an alliance, not a legal business partnership) between the Coach and the Client in a thought-provoking and creative process that inspires the client to maximize personal and professional potential. It is designed to facilitate the creation/development of personal, professional, or business goals and to develop and carry out a strategy/plan for achieving those goals.
Fee Structure
This agreement between the Coach and the Client will continue regularly under the following fee structure:
for a period of 3 months following the first coaching session.
Cancellation & Termination
Cancellation Policy
Client agrees it is the Client's responsibility to notify the Coach 24 hours in advance of the scheduled calls/meetings. Coach will not reimburse or add an appointment for the Client for a missed meeting unless agreed upon in advance.
Termination
This Agreement may be terminated by either party at any time for any reason upon 30 days written notice.
Confidentiality
Anything that is said or revealed in the sessions between the Coach and the Client is privileged information and will not be disclosed to any outside party without the Client’s written consent. The exception is if the information revealed includes disclosure of illegal, unethical, or criminal activities.
Please be aware that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege.
The Coaching Process Agreement
The methods employed by Alex Sawyer are guidelines that the Client must follow through on in order to see any type of change. The Client agrees, therefore, to hold the Coach free of any liability and responsibility for adverse reactions as a result of advice given in the coaching sessions.
I understand that coaching is not therapy nor is the coach qualified to give legal or financial advice. I take full responsibility for all actions I take as a result of coaching and that Alex Sawyer Productions LLC cannot be held responsible for any action taken or not taken as a result of coaching.
I also agree to the following:
- I agree to be fully present and on time for all coaching conversations.
- I realize I am completely responsible for my own growth and learning.
- I realize there may be times in the coaching process where I may confront realizations that may be uncomfortable.
- I realize I need not discuss anything I am not comfortable discussing.
- I realize I am responsible for my own actions and inactions.
- I realize my coaching relationship is a resource, and that it is up to me to fully utilize this resource.
- I agree to communicate any concerns I have about the nature of my coaching relationship, my experience of the coaching relationship or any particular interactions with the coach.
- I agree to give at least 24 hours notice if I need to change a session.
- I understand that all decisions in these areas are exclusively mine and I acknowledge that my decisions and my actions regarding them are my sole responsibility.
This contract is entered into for the purpose of establishing good communication and mutual understanding. Any changes or updates made to the Coaching Agreement will be agreed upon by both the Coach and the Client then formalized and confirmed via email.
The signatures below indicate agreement that the responsibilities and the information contained in this agreement are accurate.
I hereby agree with the information laid out in this document in full, and commit to the above coaching agreement presented.
